Billing Policy

We accept most major insurance plans, including Aetna, Blue Cross/Blue Shield, Coventry, First Health, Health Alliance, John Deere of the River Valley, Medicare, and United Health Care. You are welcome to contact your insurance carrier if you have questions regarding participating providers, benefits & coverage or pre-authorizations.

​We also accept payment by cash or check, credit cards and debit cards. All co-insurance, co-pays and other fees not covered by insurance are due at the time of service.

Uninsured patients are expected to pay in full at the time of service.

Payment for elective (non-emergent/non life threatening) services not covered by insurance will be due before your appointment.

Obstetrical patients will be required to set up a payment plan for prenatal care and delivery services to be paid before delivery. All co-payments and co-insurance due on ancillary tests (labs, ultrasounds) will be due at the time of service. Note that any additional tests or complications may result in additional charges and some lab services may be billed from outside laboratories.

Billing your Insurance carrier is done as a courtesy to you. YOU are ultimately responsible for all fees incurred with your care and it is important for you to be aware of your policy guidelines. If you do not provide us with the correct Insurance information it will result in delayed claims processing or denial of your claim. Please make sure you update all of your information with us regularly.
